The Problem
Northwind Health Alliance runs nine primary and specialty care clinics across two states, but patient-facing technology hadn't been touched since a 2016 website redesign. Scheduling ran entirely through a phone-based call center, appointment reminders didn't exist, and patients had no way to see lab results or message a provider outside of business hours. The clinical staff split records across two EHR platforms inherited from a prior merger, meaning front-desk teams manually cross-referenced charts for a meaningful share of visits.
The pandemic had also pushed Northwind into telehealth almost overnight using a generic video-conferencing tool bolted onto email invites, with no scheduling integration, no waiting room, and no audit trail — a compliance risk that leadership wanted resolved before their next accreditation review. No-show rates sat at 21%, and the call center was fielding over 18,000 calls a month, most of them routine.
The Solution
TechNova ran a four-week discovery engagement mapping patient journeys across all nine clinics and both EHR systems, then designed CareFlow as a HL7/FHIR-based integration layer sitting in front of the legacy platforms rather than replacing them outright — a decision that let Northwind go live without a disruptive data migration. The portal and telehealth experience were built on Next.js and Node.js, deployed on AWS within a HIPAA-eligible environment with VPC isolation, encrypted storage, and full audit logging on every chart access.
Telehealth visits were built directly into the scheduling flow using WebRTC for peer-to-peer video, with a virtual waiting room, in-session screen share for reviewing test results together, and automatic visit-note templates for providers. Twilio handled two-way SMS reminders, letting patients confirm, cancel, or reschedule from a text message rather than calling in. We worked in two-week sprints with Northwind's compliance, clinical, and front-desk staff embedded as reviewers, which surfaced workflow issues — like how proxy access should work for pediatric and elder-care patients — long before launch.
Rollout was phased clinic-by-clinic over six weeks, with a dedicated feedback channel monitored jointly by TechNova and Northwind's clinical operations team to catch friction points early.
Results
Within five months of full rollout, no-show rates fell from 21% to just under 14.5%, driven almost entirely by automated reminders and one-tap rescheduling. Telehealth visits, which previously took front-desk staff several manual steps to set up, now launch in under two minutes end-to-end, and virtual visit volume has more than doubled as a result. Call center load dropped sharply as routine scheduling and prescription-refill requests moved to self-service, giving staff time back for higher-acuity patient needs.
Beyond the immediate operational wins, the FHIR integration layer built for CareFlow is now the backbone Northwind plans to use for its upcoming remote patient monitoring program, turning what started as a patient portal into durable clinical infrastructure.
